VOTER IDENTIFICATION REQUIREMENTS
Voters must bring identification to the polls in order to verify identity. Identification may include a current and valid photo identification, a military identification that shows the voter’s name and current address, or a copy of a current utility bill, bank statement, government check, paycheck, or other government document, other than a notice of an election or a voter registration notification sent by a board of elections, that shows the voter’s name and current address.
Voters who do not provide one of these documents will still be able to vote by providing the last four digits of the voter’s social security number and by casting a provisional ballot. Voters who do not have any of the above forms of identification, including a social security number, will still be able to vote by signing an affirmation swearing to the voter’s identity under penalty of election falsification and by casting a provisional ballot.
ABSENTEE BALLOT INFORMATION
Any voter is entitled to vote by absentee ballot in Ohio without providing a reason. The application may be obtained from the Perry County Board of Elections, 200 South Jackson Street, New Lexington, Ohio, by calling 740-342-2134, or by visiting their website at https://sites.google.com/site/perrycountyelections. The application may also be obtained at the Secretary of State’s website at http://www.sos.state.oh.us/sos/elections/Voters.aspx.
Applications for absentee ballots to be mailed must be received by the Perry County Board of Elections by 12 p.m. on Saturday, Nov. 2, 2019 (three days before the general election).
Absentee ballots to be hand-carried may be applied for in person at the Board of Elections office until 2 p.m., Monday, Nov. 4, 2019. (Please note, you may only hand-carry your own absentee ballot, unless you have a medical power of attorney specific to Ohio elections on file with the Perry County Board of Elections.)
Voter, or voter’s minor child, with an unforeseeable hospitalization may apply for an absentee ballot beginning 12:00 p.m. the Saturday before Election Day and ending at 3:00 pm on Election Day.
Absentee ballots, returned in person or via a method other than U.S. Mail, and not having a postmark, must be received by the boards of elections office no later than 7:30 p.m. on Election Day. (No one may return a voted absentee ballot to a precinct polling location.)
UOCAVA ballots returned by personal delivery must be received by the boards of elections office no later than 7:30 p.m. on Election Day. (No one may return a voted absentee ballot to a precinct polling location.)
Absentee ballots returned by U.S. Mail must be postmarked no later than Monday, Nov. 4, 2019 (day prior to the general election) AND received by boards of elections by November 15, 2019 (10 days after the general election) to be counted.
UOCAVA absentee ballots must be received by boards of elections no later than Nov. 15, 2019 to be counted (10 days after the general election).
Absentee ballots must be applied for in writing. If you are properly registered to vote, you may complete an application for an absentee ballot at the Board of Elections office, you may submit your written request to the Board of Elections, you may call the Board of Elections and request an absentee request application be mailed to you, or you may complete the application mailed to you from the Ohio Secretary of State’s Office. Your written application for the absentee ballot need not be in any particular form, but it must contain ALL of the following information:
• Your name;
• Your signature;
• The address at which you are registered;
• Your date of birth;
• One of the following:
1. Your Ohio driver’s license number; or
2. The last four digits of your Social Security number; or
3. A copy of your current and valid photo identification,
Military identification showing the current name and address, current utility bill, bank statement, government check, paycheck, or other government document that shows your name and current address (Note: You cannot use as proof of identification any notice that the Board of Elections mailed to you.)
• A statement identifying the election for which you are requesting an absentee voter’s ballot;
• A statement that you are a qualified elector;
• If you want the ballot to be mailed, the address to which you want it mailed.

Election Day, Tuesday, Nov.r 5, 2019 – All voting must take place at the polls; no voting permitted at the board office.
